  page, it is right under the search engine. 

2. Print the item, quantity, size and price.

  (don't forget upcharges for XXL and XXXL sizes)
3. Total up your order and write a check to:

4. Ky State Sales Tax is already included in the
   pricing

5. Make a copy of the order form for yourself.

Items will be delivered to the soccer fields.
