Personalization is one line of customized text - **2 WORDS MAXIMUM**. Examples of this could include FirstName LastName, or FirstInitial Last Name, Last Name only, or 2-word only title.

Personalization will be embroidered Right Chest above Child & Adolescent Psychiatry (see image for example). (Duffel Bag personalization will be on side pocket.)

To purchase personalization for each garment, you must enter the quantity to match the quantity in your cart (3 garments, add 3 personalizations). Enter the quantity in the box under the price.

REQUIRED: On the checkout page, in the "Additional/Special Instructions" field, please enter your name exactly as you would like it to appear. First letter of each word will be capitalized.
